As the final stage of the instructional design process, I have experience creating surveys for program assessment and evaluation. Surveys have ranged from few questions after a webinar, to multi-page branching surveys regarding all aspects of an in-person event. Following survey closure, I have created post-event reports analyzing the data and providing recommendations moving forward.
